How are you tracking re-work/come-backs?

How are you all currently tracking re-work or work that comes back in because it wasn’t done right the first time? Do you track who did the original repair, and tracking come-backs as a performance metric? Do you track the reason for the come-back (mis-diagnosed, improper procedure, part failure, etc?)

Are you doing that in RTA, or using other means? If in RTA, what fields are you utilizing (reason codes, cause codes?)

I check on a vehicle’s history and make up the work order with the rework reason picked. Unless it was a bad part then it is under warranty. Like a alternator that only last like a week or so.
